Case Manager
Schedule: Wednesday–Sunday, 8am–5pm
Compensation: Starting at $25/hour (Full-Time, Non-Exempt)
Location: Claudia House, Portland OR (on‑site)
Position Summary
Provide client‑centered, trauma‑informed case management for justice‑involved young adults (ages 18–25) on probation/parole with the Oregon Youth Authority, supporting individualized service plans, community navigation, reentry goals, and residential stability for Claudia House Residential Independent Living Program (ILP).

Key Responsibilities
-
Develop and implement Individual Service Plans (ISP), Master Service Plans (MSP updated every 30 days), and Independent Living Plans (ILP); lead service‑plan meetings.
-
Coordinate supportive services: life‑skills training, educational and vocational support, community resources, and referrals.
-
Create and deliver Aftercare and Transition plans; provide 180 days of aftercare following program exit.
-
Coordinate mental health, substance‑use, medical, educational, and reentry services with community providers and JPPOs.
-
Lead individual and group life‑skills sessions and conduct regular youth check‑ins.
-
Transport residents to appointments, work, school, errands, and community activities.
-
Monitor progress toward goals, maintain accurate records, daily logs, service notes, incident reports, and provide OYA‑required documentation.
-
Support medication management, assist youth in establishing medical care, and manage Transitional Visits calendar.
-
Maintain a safe, inclusive residential environment; participate in trainings; serve as mandatory reporter.

Prepare or supervise meals and ensure program cleanliness and confidentiality.
Qualifications
-
Bachelor’s degree in social work, psychology, sociology, criminal justice, or a related field.
-
1–2 years’ experience working with justice‑involved youth, young adults, or other vulnerable populations; Independent Living Program (ILP) experience or reentry work strongly preferred.
-
Knowledge of trauma‑informed care, motivational interviewing, crisis intervention, and reentry best practices.
-
Experience working with probation/parole systems; Oregon Youth Authority (OYA) experience preferred.
-
Strong case‑management, documentation, organizational, and communication skills.
-
Demonstrated ability to collaborate with courts, JPPOs, community service providers, and multidisciplinary teams.

Experience facilitating life‑skills groups, coordinating vocational or educational programs, and measuring reentry outcomes preferred.
-
Proficiency with Microsoft Office or Google Docs and basic technology for documentation.

Valid Oregon driver’s license and insurable driving record.
-
Current First Aid and CPR certification or ability to obtain within hire period.
-
Must obtain a Food Handler’s card within 30 days of employment.
-
Ability to pass background checks in compliance with OYA, state, and federal requirements.
-

Direct Care Staff
Employment: Part‑time/hourly, Non‑exempt — $20/hour
Schedule: Friday–Sunday (shifts as scheduled)
Location: Claudia House, Portland OR (on‑site)

Position Summary
Provide daily supervision, support, and guidance to residents in Claudia House’s ILP program, modeling positive behavior and supporting independent‑living skill development within a safe, structured, trauma‑informed residential setting.

Key Responsibilities

Provide direct supervision to ensure a safe and supportive living environment.

Use trauma‑informed, positive youth development, and harm‑reduction approaches.
-
Assist residents with independent living skills: meal prep, budgeting, hygiene, job readiness, and homework support.
-
Provide crisis intervention, de‑escalation, and behavior management; notify leadership or emergency services as needed.
-
Administer and monitor medication per policy and state guidelines; maintain accurate documentation.
-
Support intake, orientation, transitions, and discharge processes.
-
Transport residents to school, medical appointments, work, and community activities.
-
Maintain household cleanliness and compliance with health and safety standards.
-
Participate in team meetings, trainings, and professional development; serve as mandated reporter.

Qualifications
-
Bachelor’s degree preferred but not required; combinations of education and relevant experience considered.

At least 1 year experience working with at‑risk or justice‑involved youth or young adults.
-
Knowledge of trauma‑informed care, crisis intervention, and youth development practices.
-
Strong communication, documentation, and teamwork skills.
-
Current First Aid/CPR certification or ability to obtain within 30 days.
-
Must obtain Food Handler’s card within 30 days of hire.
-
Valid Oregon driver’s license and insurable driving record preferred.
